CCSR Resource Guide 2021 coverThe Community Connections Supporting Reentry (CCSR) Resource Guide was developed to complement the information shared at the Community Connections Supporting Reentry training, and to serve as an ongoing resource to help connect individuals and families with necessary services and support to help them maintain stability, and find success in the community after incarceration. The goal of this guide is to provide simple overviews of the many services and supports available in the community, and where to go to make referrals or get more information. The CCSR Resource Guide grew over the course of the project. Updated electronic versions were made available to individuals who attended the trainings and to their organizations, as information was added to the guide.

Community Connections Supporting Reentry Partnership

CCSR 2021 logo revIn partnership with the Iowa Department of Human Services and Department of Corrections, the Center for Disabilities and Development provided technical assistance to develop and facilitate a series of trainings to bring together staff from the many service systems that support individuals reentering the community after incarceration. This included providers from substance use disorder treatment, mental health and behavioral health services, integrated health homes, community corrections, prisons corrections, and others. The trainings were one component of a statewide recidivism reduction strategy funded by a federal Second Chance Act grant. The first round of trainings were conducted in winter 2016, and were repeated in fall 2016 and again in early 2017.
